1. Start with a new file,100×50, chose white as background.

2. Then take the Single Row Marquee Tool and fill it with black #000000.

3. And then go to Filter>Noise>Add Noise and set the Amount to 400%, Distribution Uniform and Monochromatic.

4. Then press Ctrl+t and draw it all the way up and press enter.

5. Take the Rectangular Marquee Tool (M) and draw out a rectangle and press delete.

6. Then take the Text Tool (T) and type in some numbers or what ever you have in mind.

And you are done!
